Please read below Amanda Azzopardi terms and conditions upon which the services provided are based. These terms and conditions should be read before any consultation being booked. They are put in place to safeguard the practice and ensure you receive a service of the highest quality and safety. Cancellation of booking or consultation
Please provide 48 hours notice of cancellation, if this is not provided 50% of the treatment cost will be due, and if less than 24 hours notice is given of cancellation prior to appointment, 100% of the treatment cost will be due.
Cancellation due to an adverse event
If you are unable to make your appointment due to circumstances beyond your control, or your practitioner is unable to make the appointment due to circumstances beyond their control, due to an adverse event such as a car accident, or a natural event such as weather-related issues snow, floods or earthquakes. Amanda Azzopardi will not be held liable for any costs; however, will make every effort to complete the treatment at the earliest opportunity.
Certain treatments that require a large amount of time to be allocated for the procedure will require a deposit of £25-50, if you decide not to proceed with the treatment, if it was a consultation, the money can be refunded to you in 5-10 working days. If you proceed with the treatment, it will be reduced from your final amount. If you need to cancel or re-arrange your appointment, you can do this up to 48 hours’ prior to the appointment and a full refund will be allocated.
Suitability of clients for treatments
Treatments are not provided to patients under the age of 18, and the practitioner retains the right to refuse treatment to anyone for medical or psychological reasons, or any other reason the practitioner deems the patient is unsuitable for treatment, the practitioner’s decision in this respect is final.
Please do not have pets or unsupervised children with you at the appointments, the practitioner reserves the right to withhold treatment until these issues are rectified.
Gift vouchers and promotional offers are to be used within their offered time limit, once expired they will not be honoured in alternative clinics, or in conjunction with other promotional products
Smoking is not permitted inside the clinic, if I am visiting your home, I would appreciate it if you refrain from smoking immediately before, during and after treatment
Prior to commencing your treatment, a full consultation will occur, your practitioner will explain the procedure in full to you, a full medical history will be taken, your suitability for treatment will be discussed, and options offered. A full consent form will be completed and signed by the practitioner and patient, agreeing you understand the full terms and conditions of treatments, benefits, risks and complications. The client retains the responsibility for full disclosure of all medical history prior to treatment. Amanda Azzopardi will not be held liable for any complications, damage or death that occurs from the client’s failure to disclose these details. By signing this consent you are aware that all results vary per individual treatment and no guarantee can be provided of the eventual clinical outcome.
Photographs will be taken from 5 different angles prior to treatment and after your treatment is completed, to monitor treatment progress. You will be asked to sign on the consent form if you are happy for the photos to be used for marketing purposes, this is entirely your choice.
A review will be offered of your treatments 2-3 weeks post-treatment, to allow for the treatment to settle, any adjustments of filler due to the product settling unevenly will be corrected free of charge. If you decide you would like a further enhancement, this will be provided at the original cost. Correction of anti-wrinkle treatments will be dependent upon the original cost. There are two pricing structures for anti-wrinkle treatments, one at a higher upfront cost with a ‘top-up’/ review price included and one at a lower initial cost, with an additional cost for ‘top up’/review in 2 weeks’ time. You can choose to pay a higher price initially with the review price included or a lower initial cost and then an additional cost at the review.
Refunds will not be offered on services provided, however if you are unhappy with your treatment results, I would encourage you to discuss this with me, so we can discuss your treatment options.
Contact and personal records; you will be asked to sign a consent form to comply with privacy laws, requesting permission to contact you and to ask your preference for contact, your personal records will be kept safely and securely to comply with GDPR regulations
The client will be provided with a post-treatment instruction sheet and agrees to comply with all safety instructions regarding their treatment.
Reasons to Contact your Practitioner Immediately:
If you have a fever or a shivering cold
If you are experiencing extreme pain
If the area is red or hot to touch, or the opposite if the tissues are not being perfused, the temperature will be reduced
If there is blanching in an area not injected if you put pressure on an area and the blood supply is not returning immediately
If you have a visible infection
If the area is dusky or purple in colour
If you have any concerns at all, give your practitioner a call
If you are unable to contact your practitioner for any reason and are worried about your condition, present to the closest hospital for medical advice
